Player PARTICIPATION Waiver

Acknowledgment of Risk

The undersigned understands that participation in volleyball tryouts, drills, conditioning, scrimmages, jumping, diving, running, and other related athletic activities involves inherent risks. These risks include, but are not limited to, falls, collisions, contact with other participants, overexertion, equipment-related injuries, dehydration, illness, aggravation of pre-existing conditions, serious bodily injury, permanent disability, and, in rare circumstances, death.

The undersigned voluntarily chooses to allow the participant to attend and participate in the club's volleyball tryout activities with full knowledge of these risks.

Medical Fitness

The undersigned represents that the participant is physically able to participate in volleyball tryouts and has no medical condition that would make participation more dangerous except as disclosed on my tryout application.

The undersigned understands that the club, its coaches, staff, volunteers, and facility providers are not providing medical care or health insurance for the participant.

Release and Waiver of Liability

In consideration for being permitted to participate in volleyball tryouts, the undersigned, on behalf of the participant and on behalf of the participant's parents, guardians, heirs, executors, administrators, and assigns, releases, waives, and discharges the club, its owners, directors, officers, employees, coaches, trainers, volunteers, agents, representatives, sponsors, event operators, lessors, and the owners and operators of any facility used for tryouts from any and all claims, demands, causes of action, damages, losses, costs, or expenses arising out of or related to any injury, illness, damage, or loss sustained by the participant during or arising from participation in the tryouts, except to the extent caused by gross negligence or willful misconduct where such waiver is not permitted by applicable law.

Indemnification

The undersigned agrees to indemnify and hold harmless the club and the other released parties from and against claims, liabilities, damages, costs, and expenses, including reasonable attorney's fees, arising out of the participant's attendance or participation in the tryouts, to the fullest extent permitted by law.

Emergency Medical Authorization

If the participant becomes ill or injured during the tryouts, the undersigned authorizes the club and its representatives to obtain emergency medical care for the participant when the undersigned cannot be reached immediately. The undersigned accepts responsibility for any resulting medical charges.

Rules and Conduct

The participant agrees to follow all club rules, coach instructions, safety requirements, and facility policies. The club reserves the right to remove a participant from tryouts for unsafe conduct, disruptive behavior, or failure to follow instructions.

Parent/Guardian Consent for Minors

If the participant is under 18 years of age, the undersigned parent or legal guardian certifies that he or she has read this document, understands it, and agrees to its terms on behalf of the minor participant and himself or herself.
